Forms and Petitions
The free Adobe Reader is required to view and print the forms which can be:
- Downloaded from this page.
- Completed.
- Signed.
- Delivered, mailed or faxed to the Registrar's Office, an academic advising office or an extension office, as appropriate.
- Academic Petition
- Change of Registration (Drop/Add)
- Enrollment Verification
- Independent Course/Study Contract
- Postgraduate Independent Study Request
- Transcript Request
- Withdrawal Request Form
Auditing Classes (Auditor)
Auditing is defined as studying without credit.
- All audits require professor approval (by signature on the appropriate form—see forms list below)
- Submit appropriate form to the Registrar’s Office for registration
- Audits are registered only during the time period between the first day of the term and the end of drop/add for the term.
- Courses not available for audit include ExL Virtual Campus courses and private instruction courses.
